Activation Compared to Reactivation If you ordered a replacement phone or ported your phone number from a different carrier NWP will not activate your device until you request activation. (New lines of service are activated prior to shipment and do not require submission of this form.) If your device was suspended or cancelled and you want to reactivate it utilize the reactivation form. Cancelled devices can only have the phone number reactivated for up to 30 days from date of cancellation.
Use this form only to request the NWP to activate a Replacement wireless device. Do not submit until you receive your new device.

Note: any wireless device that has been cancelled longer than 30 days cannot be reassigned the same wireless number. ATTENTION to all Mobility Services Coordinators, Managers and Property Custodian/Delegates. All property information must be complete and accurate before submitting. If not, the information will result in errors during the property upload. By submitting this form to the NWP, you agree that the wireless device asset will be loaded directly into your cost center.
